Serves as the central point for coordinated case management and access to services for children and youth and ensures that youth with intensive needs receive comprehensive support services. Under the supervision on the Local Management Board Director, will serve as staff support to the Local Care Team (LCT), receive & respond to referrals made to the Local Care Team, ensure that the at-risk youth’s plan for care has been addressed, maintain confidential and accurate data and record-keeping, reports on required performance measures and resource needs, maintains, analyzes and produces written reports, maintains directory of all community-based resources, works collaboratively with divers individuals, develops and presents training modules to small and large groups, ensures compliance with LCT membership, record-keeping and operational requirements.
