The Case Manager/Program Coordinator will support the Violence Intervention and Prevention Program by providing case management services to referred youth and families along with coordinating and supervising the Community Mentoring Program.
ESSENTIAL FUNCTIONS:
- Recruit mentors for the Community Mentoring Program
- Coordinate Mentoring Services
- Identify needs of referred youth and families using strength & need assessments
- Create case management plans for referred youth & families
- Collaborate with community members and organizations to provide support to referred youth & families
- Host & attend community Events
- Collect required data & prepare performance reports
